In order to apply for a passport book or card, please read pages 1,2,4 and 5 of "The Application for U.S. Passport or Registration" (Form DS-11).  Copies of this form are available from the Information Services staff at the Kirkwood Public Library, at a designated post office, or under passports at the state department's passport page.

Some general guidelines for successful submission of a passport application are:

  1. An unsigned completed application filled out in dark black ink.
  2. Proof of U.S. citizenship, such as a state certified birth certificate, a certificate of naturalization, a certificate of citizenship or a previous valid U.S. passport. Photocopies of these documents cannot be accepted.
  3. Proof of identity: a photo or physical description with a signature, such as a driver's license or a previous passport.
  4. One color photo that meets the U.S. Passport Agency specifications outlined on page 2 of the Application and has been taken within the last 6 months. 
  5. The expired passport and a state certified birth certificate if a child age 15 years or younger is applying for renewal of the passport.
  6. Fees for a passport book:
    Processing Fee
    $110 for an individual 16 years of age or older.
    $80 for an individual 15 years of age or younger.
    Check payable to the U.S. Department of State.
    No cash or credit card.
    Execution Fee
    $25
    Cash, check or money order payable to Kirkwood Public Library.
    Visa or Master card are also accepted.
    Expedited Fee
    $60 additional per application.
    This amount will be added to the processing fee in the check payable to the U.S. Department of State.
    No cash or credit card.
  7. Fees for a passport card that may be used for international travel by land or sea between the United States and Canada, Mexico, the Caribbean or Bermuda:

    Processing Fee
    $30 for an individual 16 years of age or older.
    $15 for an individual 15 years of age or younger.
    Check payable to the U.S. Department of State.
    No cash or credit card.
    Execution Fee
    $25 per application.
    Cash, check, or money order payable to Kirkwood Public Library.
    Visa or MasterCard are also accepted.

Any international travel by air requires a passport book.

DO NOT SIGN THE APPLICATION UNTIL REQUESTED TO DO SO BY THE PERSON ADMINISTERING THE OATH.
